Benign Prostatic Hyperplasia (BPH)
Benign prostatic hyperplasia (BPH) is the non-cancerous swelling of the prostate gland which affects nearly all men to some degree, beginning in late middle age and leading to such symptoms as:

- nocturia (the need to get up in the middle of the night for a trip to the bathroom),
- frequent and sudden urges to urinate even when the bladder if not full,

Yes, there IS evidence that Saw Palmetto - the most common herb for BPH - and other botanicals are helpful for improving the SYMPTOMS of prostate enlargement (BPH); unfortunately, they are only short-term symptom relievers.
The German "Commission E monographs" are careful to spell this out: Saw Palmetto “relieves only the symptoms associated with an enlarged prostate without reducing the enlargement. Please consult a physician at regular intervals.”
But (and this is a big “but”) when it comes to the underlying disease process itself, there is absolutely NO evidence that ANY of the standard prostate botanicals actually reduces the SIZE (volume) of the enlarged prostate.
In other words, these common botanicals fail to address the underlying PROBLEM, as opposed to its SYMPTOMS.
Chronic Prostatitis (CP)
Unfortunately, BPH is not the only prostate disorder that men may face. Another is chronic prostatitis (CP), an ongoing inflammation of the prostate gland, reflected in the presence of markers of inflammation in the prostate fluid.
Prostatitis is sometimes caused by recurrent bacterial infection, but is often present in the absence of such “nasties;” at least some prostatitis may actually be caused by unusual muscle tensions at the base of the pelvis. In this case, behavioral therapy can be of some help to ease the symptoms where this is the root problem.
Prostatodynia
There is also prostatodynia, which is distinct from chronic prostatitis (CP) in that the chemical markers of inflammation are not seen in the prostatic fluid.
Because the symptoms of these disorders - prostatodynia and prostatitis - sound similar, many men with chronic prostatitis or prostodynia self-medicate with Saw Palmetto - with the result that their symptoms remain and their health problem goes untreated.
It should be stressed that prostatodynia is a type of inflammation of the prostate NOT due to bacterial infection and in which there are no objective findings, such as the presence of infection-fighting cells, in the urine of men who suffer from the disorder.
Unfortunately, prostatodynia is typically a chronic, painful disease. The symptoms - including chills, fever, pain in the lower back and genital area, body aches, burning or painful urination, and the frequent and urgent need to urinate - characteristically go away and then come back without warning.
The urine and fluid from the prostate reveal no evidence of a known infecting organism or of cells that the body usually produces to fight infection.
Standard medical treatment - therapy with antibiotics and with drugs that relax the muscles of the prostate gland - is ineffective and fails.
Chronic Prostatitis and Prostatodynia: Open Trials
Several open trials have found the DP Extract to be helpful for chronic prostatitis and prostatodynia.
Rugendorff et al ran one such trial, in which 72 men with chronic prostatitis or prostatodynia uncomplicated by prostate stones or non-prostatic complications like blockages of the bladder “neck” were administered the DP Extract.
Examination by digital rectal exam, urine flow, and white blood cell count along with other immune markers found that 78 percent of these men were helped by DP Extract.
A second group of men whose chronic prostatitis was complicated by the factors mentioned above were not found to benefit, however. [Rugendorff et al, “Results of treatment with pollen extreact (cernilton N) in chronic prostatitis and prostatodynia.” Br J Urol. 1993 Apr; 71(4): 433-8].

DP Extract Vs. Proscar®
There is plenty of evidence that finasteride (Proscar®), the most famous DRUG therapy for enlarged prostate, can reduce prostate volume. Unfortunately, Proscar®:
- is slow to take effect (one year minimum!);
- does not work with several classes of patients;
- is very expensive, and
- has significant side effects, including erectile dysfunction and loss of libido.
So men are left with a poor choice:
- herbs which provide symptomatic only improvement with no halt to the loss of prostate health, or
- a drug treatment which addresses the core problem, but causes problems of its own (side effects).

DP Extract: Evidence
Many people find it strange that something as simple as flower pollen could have powerful effects on prostate health. And yet the clinical evidence is plain: standardized pollen extract quickly improves prostate symptoms and reduces prostate volume (reductions in the volume of the prostate are measured by ultrasound).
For example, in a massive open-label observational study on men with several prostate disorders, including 1,116 with BPH, those men with BPH and chronic prostatitis experienced a 55.9% reduction in prostate volume while on the DP Extract, along with decreases in residual urine, increases in urine flow rate, and greater total urine volume with decreased time taken to empty the bladder. Both patients and physicians rated the average improvement “good to very good.” [Schmiedt et al (eds), Therapy of Prostatitis, (Munich: Zuckerschwerdt Verlag, 1986): 154-160].
Several successful open trials of the efficacy of DP Extract against BPH have been performed. One trial, for example, found that the DP Extract group demonstrated greater improvements in subjective symptoms, painful urination, and frequent urination. DP Extract: The PSA and PAP Trial
This trial from 1986 also measured the levels of prostate-specific antigen (PSA), a marker used to detect prostate cancer, along with prostatic acid phosphatase (PAP), a measure normally elevated in many prostatic dysfunctions.
In the DP Extract group, both PAP and PSA were significantly reduced.

There has been exciting progress made in the discovery of natural ways of reducing the risk of prostate cancer, including successful double-blind, placebo-controlled trials with:
- selenium (mineral),
- alpha-tocopherol (vitamin E), and
- lycopene (the carotenoid ).

Later studies suggested that also other active compounds in the pollen extract may play role in its anti-prostate cancer effect. Whatever the true active ingredient, however, one thins is clear: defined pollen extract may prove to be potent nutritional support against prostate – and perhaps other – cancers.
In this context, the lowering of PSA levels experienced by men receiving the DP Extract is very tantalizing. [Bräuer, "The treatment of benign prostate hyperplasia with phytopharmaca. A comparative study of cernitin vs. beta-sitosterol." Therapiewoche. 1986; 36: 1686-96].
But we cannot be certain that his means a reduction in risk of prostate cancer. Proscar®, for instance, also lowers PSA, but does not appear to have any effect on this terrible disease.
Thus, it is possible that his lowering of PSA may reduce the usefulness of the PSA test as a marker for prostate cancer (as is known to happen with Proscar®).
Therefore, as with Proscar®, you should have your PSA checked before starting on the DP Extract, to establish ha baseline from which future test can be evaluated.

Aspirin and Ibuprofen May Interfere With Prostate Exams (Study)
Common over-the-counter painkillers may interfere with the detection of prostate cancer, according to a study published in of the journal Cancer (2008, the October 15 issue).
The study found that regular use of NSAIDs or nonsteroidal anti-inflammatory drugs, such as aspirin, ibuprofen (Motrin, Advil) and naproxen (Aleve), among others, may reduce levels of the prostate biomarker prostate-specific antigen (PSA).
Tylenol (acetaminophen) also had a similar lowering effect, but not to the same degree.
After surveying 1,319 men over the age of 40, researchers found that regular users of pain killers had PSA levels about 10 percent lower than those who do not regularly use them.
In other words, taking an aspirin a day could muddle test results because increased levels of PSA are considered a possible indication of prostate cancer, according to the study.
Although NSAIDS lower PSA levels, researchers said it is unclear whether they offer protection against prostate cancer as suggested in previous studies. PSA screening may also be turning up tumors that pose no threat to a man's life.
Larger ongoing clinical trials currently underway in the U.S. and Europe may answer whether regular PSA screening necessarily saves lives. Prostate cancer kills about 250,000 men a year worldwide.
"Given the widespread consumption of NSAIDs, known for their high-anti-inflammatory effect reducing pain, fever and inflammation, and the regular use of PSA for the assessment of prostate cancer risk, the potential implications of our findings may be substantial and warrant further investigation," the authors wrote in their study.
Researchers conclude that doctors should ask patients about all medications they are taking before a screening.
PSA screenings are usually done in a doctor's office and test for PSA, a protein produced by cells in the prostate gland. Elevated levels may indicate prostate cancer.

- Ureaplasma urealyticum - a species of detrimental ureaplasma bacteria, a genus of bacteria belonging to the family Mycoplasmataceae. Mycoplasma, also known as Asterococcus, is a genus of aerobic to facultatively anaerobic, gram-negative detrimental bacteria. Ureaplasma urealyticum infection may cause urinary tract infections (UTIs), urinary tract/kidney stones (struvite stones), non-specific urethritis (NSU), nongonorrheal urethritis and prostatitis in men. It has also been noted as one of the infectious causes of sterile pyuria (pus in urine).

P. aeruginosa is the third leading cause of hospital-acquired urinary tract infections, accounting for approximately 12 percent of all infections of this type. They are usually related to urinary tract catheterization, instrumentation or surgery.
It is a common cause of post-operative infection in radial keratotomy (RK) eye surgery patients and of "hot-tub rash" (dermatitis), caused by lack of proper periodic attention to water quality.
P. aeruginosa appears to be among the most adherent of common urinary pathogens to the bladder uroepithelium. An infection can occur via an ascending or descending route.
P. aeruginosa can also invade the bloodstream from the urinary tract. This route is the source of approximately 40 percent of P. aeruginosa infections.
P. aeruginosa can cause chronic opportunistic infections. These kinds of infections are a serious problem for medical care. Many patients, immunocompromised and the elderly in particular, often cannot be treated effectively with traditional antibiotic therapy.
As a highly relevant opportunistic pathogen, P. aeruginosa has low antibiotic susceptibility. It is naturally resistant to a large range of antibiotics and may demonstrate additional resistance after unsuccessful treatment.
Therefore, choosing an antibiotic should be guided according to laboratory sensitivities, rather than empirically. However, if antibiotics are started empirically - with some exceptions, given by injection only - then cultures should be obtained and, after having the culture results available, the choice of an antibiotic used should be carefully reviewed.

Recommended Intake
